The Blue Advantage^® Care Management team helps patients navigate the healthcare system and offers care coordination for optimal healthcare outcomes including preventive and condition-specific education, medication reconciliation, assessing for any barriers to care and individual needs discussion. Care Management is a voluntary program that is part of your patients’ health benefits at no additional cost. Once enrolled, your patient is assigned to an experienced team that includes a nurse, social worker, pharmacist and medical director.

The Care Management team can connect your patients to valuable pharmacy services that are included in the Blue Advantage benefits to help ensure that prescribed medications are taken regularly to maintain their overall health. 

Social Workers are available to help assist patients with needs such as access to healthy food, obtaining durable medical equipment, medication assistance programs if patients cannot afford their medication, transportation to and from doctor appointments and financial concerns.
