Health Risk 360 (HR360)

The Health Risk 360 (HR360) is an important assessment that helps to capture your Blue Advantage® patients’ full picture of health, including chronic conditions and medications. In 2024, providers are required to complete and submit an HR360 for each Blue Advantage patient’s Annual Wellness Visit (AWV) or annual physical exam. Eligible providers can earn $150 for each accepted HR360 assessment performed for any eligible Blue Advantage patient during the calendar year.

Important Details About the HR360 Incentive:

  • New for 2024: Providers are required to complete the HR360 when conducting an Annual Wellness Visit (AWV) or annual physical exam for their Blue Advantage patients.
  • Telehealth: The encounter associated with the HR360 form’s completion must be either in person or performed via telehealth using both audio and video technology (only for AWVs).
  • Patient Eligibility: The HR360 can be completed only once per year for an eligible Blue Advantage patient per tax ID.
  • HR360 Submissions: You must use our HR360 form or the new AutoHR360 online application (see details below). Patrius Health will not accept HR360s via fax or mail. 

 

New AutoHR360

Patrius Health offers an interactive AutoHR360 application that features pre-populated data fields, which will greatly reduce the time required to complete the assessment. Review the prefilled information and update it if changes are needed. You can then submit the HR360 form directly from the application. There’s no need to upload other forms or additional medical records.

You will find the AutoHR360 link under Provider Tools in the menu at the top of the Patrius Health provider website. Refer to the AutoHR360 Provider Guide for details about using this application.

The AutoHR360 features the same fields as the HR360 form. The left-hand side of the application will guide you as you complete each section of the online form. Red numbered alerts indicate potential missing information. All fields shown in the HR360 should be filled out. Submit directly from the tool or save and submit later.  

You also have the option with the AutoHR360 to print a blank form and upload it if that works best for you. Note: For a limited time, forms can be uploaded through the medical records request in myNotifications.

HR360 FAQs

When can the HR360 be performed and completed for this incentive?
HR360 assessments may be performed during an encounter for dates of service from January 1 through December 31 of a specific calendar year.

  • Your HR360s must be submitted by midnight on January 31 of the following calendar year. For example: The HR360s for 2023 must be uploaded by midnight on January 31, 2024.

Are reports available to help providers with this incentive?
Patient attribution reports will soon be available to Patrius Health primary care providers to assist with scheduling wellness visits and identifying gaps in care. These reports are updated at the beginning of each month and show important information about a provider’s Blue Advantage patients, including:

  • Total number of attributed patients
  • Patient details (name, subscriber ID, date of birth)
  • Attribution thru dates
  • Gaps in care 
  • AWV status of attributed patients

We encourage providers to check these reports regularly to proactively schedule Annual Wellness Visits and annual physical exams. Links to these reports will be available under the Provider Tools menu at the top of the Patrius Health website.

Also Available Now: Check the Patient Health Snapshot (PHS) before each wellness visit encounter to help identify gaps in care and other important information about your patients. 

Can I perform the HR360 via telehealth?
Telehealth visits must be provided using both audio and video technology, and appropriate codes related to these services should be filed on the corresponding claim. The HR360 form requires the providers to indicate if the visit was performed via telehealth. 

Telehealth HR360 claims should also include appropriate HCPCS codes and/or modifiers for the encounter with the appropriate place of service code. Body mass index and blood pressure results for the patient will not be required for telehealth claims related to HR360. Telehealth encounters performed via audio only are not eligible for the HR360 incentive. 

Annual physical exams cannot be performed via telehealth.
